Where do loop diuretics act?

The thick ascending limb, on the Na-K-2Cl cotransporter.

Where do thiazide diuretics act?

The distal convoluted tubule, on the Na-Cl cotransporter.

Where do potassium-sparing diuretics act?

The collecting duct, on principal cells.

Where do carbonic anhydrase inhibitors act?

The proximal tubule.

Which diuretic class causes hypokalaemia most reliably?

Loop diuretics, with thiazides close behind.

Which diuretic class can cause hyperkalaemia?

Potassium-sparing diuretics such as spironolactone and amiloride.

Which diuretics raise calcium reabsorption?

Thiazides. Loop diuretics do the opposite.

How do osmotic diuretics work?

They stay in the tubule and hold water with them, mainly in the proximal tubule and descending limb.

Which diuretic class is used first in acute pulmonary oedema?

Loop diuretics, for their speed and their venodilating effect.

What is the mechanism of spironolactone?

It blocks the aldosterone receptor in principal cells.
