Before you see a card, its sentence is matched against the file. Curly quotes and line breaks are forgiven. A sentence that is not there is not.
A deck missing a third of a lecture looks complete. So the slides that produced no card are listed by title before you add a single one.

A generated card has to quote a sentence that is actually in your file, and the quote is checked before the card reaches you. A card that fails that check is not made. A card with a real sentence but a poor question is what the fix and skip buttons are for.
Yes. Slides exported as a PDF work the same as a chapter. Page numbers are kept, so each card cites its slide, and slides that are only a diagram are set aside rather than reported as missed.
