Physiology
Physiology is chains of cause and effect. A card that asks for a number teaches the number. A card that asks what happens next teaches the mechanism, and the mechanism is what the exam asks.
Most physiology sentences have the shape "when X, Y, so Z". Blank the middle and you have a card that tests the mechanism without giving it away. "The descending limb is permeable to water but not to solutes, so the filtrate [concentrates] as it descends" is a better card than "what does the descending limb do", because the sentence carries the reasoning and the blank carries the test.
Two blanks in one sentence make two cards, each scheduled on its own. Blank the cause on one and the effect on the other and you have the mechanism from both ends.
The failure mode in physiology is not the wrong fact. It is the plausible wrong mechanism: a card that says the collecting duct does what the proximal tubule does, and reads fine. You will review it for a month before the exam tells you.

Take a 24-slide renal lecture as a scenario. You write a cloze card for each of the four from the slide text, add the nineteen, and the deck is twenty-three cards that cover the lecture instead of nineteen that look like they do.
Thirty to sixty cards a lecture is the usual range for physiology. FSRS spaces them by how well you know each one, so the mechanisms you understood on the day fade out of your reviews and the ones you did not stay close. The due forecast on the deck shows the week before it arrives.
